Tell Tale Signs
This roof is leaking, and has been for awhile. By the time the water hits the drop ceiling and shows through, it's very possible that your deck is already compromised.
Flat roofs often have ponding water on the roof. It's not supposed to happen, but it's often hard to help, especially if you're working on an older roof. On this roof, it appears this newly applied roof wasn't installed correctly. The roof should taper to the drains.

This roof has two drains, yet the ponding is consistent. Ponding is never good for a roof - it eats at all joints in the roof, not to mention the algae can break down the actual material over time.
The Primary Culprit
This appears to be the main leaking area. When one finds a hole this bad, there's no telling how many more holes are in this TPO system. It's important to have a qualified contractor do your TPO project, unless you want to end up with these issues.
Exhaust Pipe Twisted Up
This pipe should have been replaced and flashed appropriately. It's too rusty to combat the elements anymore.
